Job Details:
- provide clinical and case management care to patients in an outpatient setting

Requirements:
-Master's degree required in social work, mental health counseling or related field such as art therapy

Case Manager, Licensed Residential Program, Queens

Responsibilities:
• Conducts home visits on a regular basis for assigned case load.
• Obtains psychosocial histories and conducts functional skills assessments.
• Establishes and maintains liaison with primary provider of psychiatric care.
• Assures access to psychiatric, substance abuse and medical services, and entitlement benefits.
• Monitors client medication, including adherence and side-effects.
• Develops an individualized rehabilitation plan (Service/Support Plan) in accordance with client needs, strengths, and goals and in consultation with the treatment team and others of the client’s choice.
• Delivers community rehabilitation services according to individual need, including: Assertiveness/Self Advocacy Training, Community Integration Services/Resource Development, Daily Living Skills Training, Health Services,
• Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management.
• Documents client response to service, progress and regressions, and regularly updates the Service/Support Plan.
• Trains clients in safety procedures and conducts fire drills and self-preservation tests.
• Prepares residents for transition to independent living and makes necessary referrals.
• Facilitates crisis intervention in the event of an emergency.
• Staffs Emergency Pager on a rotating basis.
• Maintains client charts and submits necessary notes and reports, including incident reports, in compliance with agency procedures.
• Oversees maintenance of apartments, inventories furniture, and alerts program director of maintenance problems.
• Attends in-service training as requested by supervisory personnel.
• Provides coverage as needed for other staff on the program team.
• Serves as “Qualified Mental Health Staff Person”

Qualifications: M.A. in psychology or a human services related field or a B.A. in psychology or a human services related field and significant experience in the field.

Cook, Licensed Residential Program Queens

Responsibilities:
• Responsible for shopping and meal preparation
• Responsible for overseeing set-up and clean-up of the kitchen and dining room at meal times

Custodian/Maintenance Worker, Licensed Residential Program Queens

Responsibilities:
• Cleans physical plant including office areas, bathrooms, kitchens, resident
• rooms, lounges, mechanical areas, elevators, stairways (windows, blinds, floors, walls, baseboards, doors, furniture)
• Buffs floors
• Cleans and maintains equipment/appliances (refrigerators, air conditioner units, etc.)
• Cleans and organizes supply and resident storage areas
• Performs snow removal
• Alerts supervisory personnel to furniture and appliance issues
• Performs light maintenance and preventive maintenance
• Alerts Program Directors and supervisor to maintenance problems
• Assist with client move-ins and move-outs

Qualifications: Able to lift at least 50 pounds, move furniture, carry equipment and climb stairs with ease. Ability to work well with others.

Front Desk Monitors, Licensed Residential Program, Queens

Responsibilities:
• Monitoring the entry and egress of building residents and guests.
• Responsible for the distribution of residents’ mail.
• Responsible for taking telephone messages for the residents and staff of the building.
• Assists with client cooperative meal plan.
• Is the first line of defense in case of emergencies; (i.e. calling management, the fire department in case of a fire, etc.)
• Monitors and documents client medication

Qualifications: High school diploma with some undergraduate work in psychology or high school diploma with significant experience in working with adults with mental illness.

Program Specialist, Licensed Residential Program Queens

Responsibilities:
• Teaches skills for maintaining personal hygiene, laundry and apartment cleaning.
• Monitors overall maintenance of apartments and alerts program director of maintenance problems.
• Teaches skills for budgeting, shopping, meal planning, preparation and clean-up.
• Teaches skills for self-preservation, monitors living units for safety and cleanliness.

• Administers the client cooking safety test.
• Monitors, documents and teaches self administration of medication, according to program policy and procedures.
• Report incidents or behavioral changes in residents to supervisor.
• Teaches travel skills and escorts clients, where needed.
• Documents services rendered and client progress in a timely manner.
• Attends training as requested by supervisory staff.

Fee-For-Service Psychologist/Social Worker

OVERVIEW:
The Social Worker/Psychologist is responsible for providing cultural competent compassionate clinical care. As a part of the therapeutic team, the worker must work closely with Pride’s medical, psychology/social work and administrative staff, to ensure the delivery of effective and efficient behavioral health services.

RESPONSIBILITIES include but are not limited to:
1. Providing individual, group and family counseling
2. Conducting intake assessments
3. Formulating Psychosocial Assessments, Mental Status Exams, treatment plans, and treatment plan reviews
4. Routinely liaise between agency and consumer's family members, relevant community members and service providers.
5. Provide case management and case coordination services, including making necessary referrals when necessary
6. Participate in case conferences, supervision, incident reviews, team meetings, and trainings when necessary
7. Obtain necessary billing authorizations, to discuss fees and billing issues with clients, and collaboratively work with the clinics administration to help resolve billing related issues that may arise
8. Participate in audit preparation when necessary
9. Complete any and all paperwork associated with the delivery of care in a timely manner
10. Other duties as assigned

REQUIREMENTS:
Masters degree in Social Work, (MSW), PhD or PsyD in Psychology with a minimum of one year of relevant experience with a similar population. LMSW, LCSW, or Psychology license required. Candidate’s bi-lingual in English and Asian Pacific Islander dialects preferred.

• Engage in research projects, outreach, and educational activities as assigned by supervisor
• Participate in program team meetings and relevant trainings.
